🧾 Key Responsibilities

• Perform patient assessments and develop care plans based on physical, psychological, social, and environmental needs
• Manage a caseload and provide comprehensive care planning, coordination, and evaluation
• Conduct initial and follow-up home visits as needed
• Communicate changes in health condition to supervisors, physicians, and care team
• Document care activities per HIPAA and Elara standards
• Revise care plans based on patient conditions
• Maintain confidentiality and comply with all health and safety regulations
• Support and align with Elara Caring’s vision and leadership

✅ Qualifications

• Current, unrestricted RN license in New York
• Minimum 1 year of nursing experience in a clinical care setting
• Valid driver’s license, auto insurance, and dependable vehicle
• Willingness and ability to travel within coverage area (up to 50%)
• Associate or Bachelor’s Degree in Nursing preferred
• Home Health or Hospice experience preferred
• Familiarity with Home Care HomeBase software preferred
